Looks like I am also seeing this with DVROnTime. All c4 and c4 HD recordings failed last night (about 1 min long) so switched the channels to padding for now. Any ideas if 4seven is affected too?
So far its been a one off but I am hoping @lc200 can make some improvements so that even if it cannot recover from the failure on the affected recording that at least any subsequent recordings on the same channel will remain unaffected...

I think I ticked the option in DVROnTime when I first evaluated it as coming from a Humax background I was aware it's files were actually M2TS as Black hole already pointed out.
I have asked them to consider adding Kodi style split Movie naming, as thats what I use for DVROntime recordings, i.e:-
Movie title (year).part1.m2ts
Movie title (year).part2.m2ts
