The fix I proposed above does resolve the favs 1,2,4 only working, however the other issues are more complex.

Having used Detectads with my padOut mod for a number of programs now I feel there is just one tweak needed, the 1st break adjustment needs to be 3secs not 2secs
ie both set padding 2 should be set padding 3

I`ve had another failure, though I don't think its related, this time queue states plugin failure.
The file is a timeslip one (it did not need processing, but got grabbed anyway) I tried several times and background process failed the same each time, error seems to be:

I agree, I`ve has numerous PVR's and the Humax is still the best, even without the CF, I had a Panasonic that had an awful clunky interface and a Technisat that liked to corrupt files, both those boxes would crash periodically, that seems to be a 'feature' of PVR's!
I did some experiments on this, I found the fastest way was via LAN using both SAMBA and FTP (I was transferring to a 2nd box) so I used SAMBA to read and FTP to write (of course if destination drive is installed on the PC that's not needed) I was using a linux machine, so just had 2 windows...
Checked again still nothing on the favourite I had created (number 3) but they are there under favourite 4, so why is the box favourite numbering different? I`ve now made a 4th favourite list on the box, it will be interesting to see where that appears on the webif.

Having added the option to choose the favourite from directly within the EPG I decided to create an additional favourite list, so did that on the box yesterday but noted the change is not reflected within the webif.
Switch on today and its still not there! I`ve applied service epg restart but...
I have created that addition and the escape key option (any OS), I will create diffs for those and all the previously listed changes to simplify things in due course.
Sorry the file is deleted, I`ll add the -d option and post if/when it re-occurs. I've been having this issue for ages and well before I made any changes to the code, I can't see they would have any effect on this anyway.
