There's a problem with the auto-processing. This particular queued item failed, and had the status "failed" in the queue, but the scheduler saw it was in the queue and wouldn't re-submit.
The queue system is modular and if a module returns FAILED then the item stays in the queue but marked as failed and needs to be manually addressed. I assume in the case you reference that the module did not return that status and there may be a bug there (or an improvement that could be made).
