Full install needed (again)

This morning, powered on … and it started with the full set up .. SCREEN. 1 ..language selection, etc.
Loses all configuration & schedules.
What causes this, happened a few times now.

HD diagnostics attached.. also attach latest log entry …. It shows I shut it down at 0038
The system booted at 04:17 … no idea what this is, nobody was using the box at that time.

Is this the startup reverting to the Installation Wizard you're talking about?
If so, I don't think it's necessary to do a full install, but there are a few, maybe up to ten, questions to answer.
It may be useful to know which version of the custom firmware you're currently on, if it's been recently updated etc.

04:17 is the usual OTA firmware update search.
Cause of the Install Wizard is suspected to be either corrupt database in the flash storage or corruption of the flash filesystem itself.
Install the System Flush image from the Wiki to remake the flash filesystem - you will lose everything stored in the flash and will need to run the fix-flash-packages diagnostic to restore it, as well as any configuration settings therein e.g. for ir, tunefix, newk, undelete, fan, boot-settings, dlna-servername and any custom encryption key.
The hard disk is irrelevant, as is the CF version, although 3.14 is recommended.

I have packages installed : IR, tunefix, newk, undelete, boot-settings they show as currently installed, do I have to do anything to them ?

Configure their settings as required. It depends what you had done to them before, but only you will know that.
