I don't use it myself, but do you have Auto-Dedup enabled?, there are some notes HERE showing how files are re-named

Yes, that sounds like auto-dedup. Not all programmes/series are amenable to the dedup process so it's generally best to try manual dedup runs until you're happy to enable the automatic version.
