Could unplugging HDMI be relevant - last crash had been 8 days previously when HDMI also disconnected?

Perhaps, but I don't recall it being mentioned before.

HDD not powering down when putting into standby has been noted previously after crashes.

There are a few things which might prevent or extend completion of the shutdown housekeeping and thereby leave the unit in "delinquent half-awake" – file errors being one.

Recording two programmes at the same time seems to create a vulnerability to crashes.

...which is strongly indicative of disk problems or overheating.
